Case Manager

ROLE: The Case Manager plays a pivotal role within the client operations team, holding direct responsibility for overseeing a designated caseload. Primary objectives include facilitating seamless service delivery, ensuring appropriate staffing, and offering support and guidance to families and providers on the team. WHY JOIN US? Provide guidance and support to the providers of the team to enable them to provide optimal care to the families. Proactively address and resolve service-related issues and challenges that may arise. Ensure the prompt receipt of patient progress reports. Actively participate in facilitating the acquisition of insurance authorizations. Monitor and work towards increasing the utilization and ensure authorized hours are effectively utilized. Collaborate with recruiters to efficiently hire staff and ensure timely and suitable case staffing. Perform candidate interviews and maintain ongoing communication with successfully hired candidates throughout the entire onboarding process. Thoroughly record and document all interactions and correspondence with both clients and providers. Ensure systems and client databases remain up to date. Participate in team meetings and integrate suggestions that are provided for the betterment of the department. Adhere to all company policies and procedures, uphold ethical and professional standards, client confidentiality, HIPAA compliance, and comply with all relevant local, state, and federal laws regarding case management provision.
