Registered Nurse - RN

Role Summary: In this role, you will be a key member of the care team, delivering personalized nursing care, coordinating with healthcare professionals, and promoting overall patient health and wellness. Duties / Responsibilities: Provide high-quality, patient-centered nursing care, including assessment, planning, implementation, and evaluation. Administer medications, treatments, and procedures according to established protocols and patient care plans. Monitor patients’ conditions, document findings, and report changes to the care team promptly. Educate patients and their families on disease management, health promotion, and preventive care. Collaborate with the care team, physicians, and other healthcare professionals to coordinate and optimize patient care. Assist in the development and implementation of individualized care plans for patients with complex needs. Maintain accurate and timely patient records, ensuring compliance with all regulatory and organizational requirements. Provide support for both in-person and telehealth visits, as applicable. Ensure adherence to infection control, safety, and HIPAA regulations. Participate in continuing education and professional development opportunities. Perform other related duties as assigned. Required Qualifications: Active Registered Nurse (RN) license in CA. Bachelor’s or Associate’s Degree in Nursing from an accredited program. Strong clinical, assessment, and patient care skills. Excellent communication, interpersonal, and patient education skills. Ability and willingness to travel to patient locations, if required. Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, if applicable. Valid driver’s license. Desired Qualifications: Experience in home health, community care, or outpatient nursing. Ability to work collaboratively within a multidisciplinary care team.
