The Role
We are looking for an experienced Bilingual Coordinator, Client & Caregiver Support with at least 2 years of experience in a home care agency or healthcare office setting. This is an in-person position for someone who is organized, proactive, solutions-oriented, and comfortable working in a fast-paced environment.

The Coordinator plays an important role in maintaining continuity of care by coordinating caregiver schedules, communicating with clients and caregivers, addressing coverage needs, and ensuring accurate documentation.

Using Axxess, our scheduling and care management system, the Coordinator will maintain caregiver availability, coordinate schedules, monitor service coverage, and assist with day-to-day client and caregiver needs. Knowledge of Electronic Visit Verification (EVV) requirements and home care operations is essential.

Responsibilities

• Serve as a primary point of contact for clients and caregivers, answering calls, relaying messages, and responding to scheduling and service-related needs
• Coordinate and maintain client and caregiver schedules in Axxess
• Confirm client service schedules and maintain accurate caregiver availability
• Schedule, adjust, and coordinate shifts to help ensure appropriate and uninterrupted client coverage
• Assist with last-minute call-outs, schedule changes, and coverage needs
• Communicate schedule changes and other important information promptly to clients, caregivers, and appropriate team members
• Maintain accurate documentation of client and caregiver communications, scheduling updates, and service-related concerns
• Monitor EVV activity and assist with identifying and resolving visit verification or scheduling discrepancies
• Follow agency policies, procedures, and applicable EVV requirements
• Gather and communicate client feedback, concerns, and changes in service needs to the appropriate team members
• Assist applicants and new caregivers with in-person applications and onboarding as needed
• Provide general office and administrative support, including filing, data entry, and handling correspondence
• Work collaboratively with nursing, HR, billing, and other departments to support continuity of care and agency operations
• Maintain confidentiality and professionalism when handling client, caregiver, and agency information

Qualifications

• High school diploma or equivalent required
• Minimum of 2 years of experience in a home care agency, healthcare office, or similar healthcare setting
• Bilingual in English and Spanish, both written and spoken
• Experience with home care scheduling and caregiver coordination preferred
• Knowledge of EVV systems and requirements
• Experience with Axxess or similar home care scheduling software strongly preferred
• Strong communication and interpersonal skills
• Excellent organizational skills and attention to detail
• Ability to prioritize multiple responsibilities and respond effectively to changing scheduling needs
