Key Responsibilities\n

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring all services align with client goals.

• Monitor client progress by regularly assessing outcomes and adjusting care strategies as needed, collaborating with interdisciplinary team members.

• Provide emotional and psychological support to seniors and their families, facilitating discussions around coping strategies and resources.

Qualifications\n
