Acute Care Case Manager

This is a 13-week contract position.

Day Shift: 5x8 Hours (08:00 - 16:30)

We are seeking an experienced Acute Care Case Manager for an immediate position. The ideal candidate will be seasoned with a strong background in acute care, case management, and discharge planning within a hospital setting. This role requires engagement in care coordination and close collaboration with multidisciplinary teams to ensure optimal patient outcomes.

Responsibilities:

• Assess patients' needs and develop coordinated care plans.
• Manage discharge planning processes to ensure safe transitions.
• Participate in family meetings and facilitate communication between patients and their care team.
• Conduct utilization review and manage prior authorizations.
• Coordinate with various departments including Social Services and Physical Therapy.

Required Experience / Certifications / Licensure:

• Minimum of 2 years of Acute Care Case Management experience required.
• California state license is necessary; pending licenses accepted if fully licensed by start date.
• Experience with healthcare organizations including HMOs or IPAs is a plus.
• Competency in computerized documentation systems, particularly Epic.
