Work Environment

This position will primarily operate onsite in the patients' homes across various neighborhoods in San Francisco. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions to determine their needs and develop personalized treatment plans.
• Implement targeted therapeutic techniques to improve mobility, relieve pain, and rehabilitate injuries.
• Regularly assess patient progress, modifying treatment interventions as necessary to achieve optimal outcomes.
• Educate patients and their families about therapeutic practices, home exercises, and safety measures to promote recovery.
• Maintain detailed and accurate documentation of all patient interactions, assessments, and treatment progress for compliance and continuity of care.
• Adhere to safety protocols and best practices, including universal precautions, to ensure a safe environment for patients and staff.
• Participate in performance improvement initiatives and contribute to a culture of continuous learning and clinical excellence.
• Collaborate with interdisciplinary teams, including physicians and healthcare professionals, to provide holistic care to patients.

Patient Population / Client Focus

The clientele served by Adage Physical Therapy Clinic includes a diverse group of individuals with a variety of conditions, ranging from post-operative rehabilitation to chronic pain management and geriatric care. Qualifications

• Current and valid Physical Therapist license in the state of California.

Preferred Experience
