Case Manager - Benton ARFull time 40 hours per weekMonday-Friday scheduleAdult program manageable caseloadCompetitive salaryGenerous PTO amountsPaid holidaysHealth vision and dental insurance options401k with company matchSUMMARYThe Case Manager meets and works with individuals interdisciplinary teams and families/guardians to gather information; assess each individuals needs; assess service needs; prepares a plan of care; and identifies and implements services to meet an individuals JOB FUNCTIONSTo perform this job successfully an individual must be able to satisfactorily perform each essential function listed below:Provides case management services including intake assessment crisis intervention advocacy referral and ongoing QIDP responsibilities (if required).Gathers social history to assess strengths and weaknesses and to assess existing individual home visits and office base services and accompanies individuals served as care with Program Directors Supervisors DSPs medical professionals nutritionists and other service providers to prevent service duplication and to ensure the individual receives high quality and procures group services appropriate for the individual to maximize their ability to live well in their home and community. Plan and assist in obtaining services from third-party service providers as a plan of care for each individual to include services such as advocacy/case management independent living skills behavioral support training peer support physical therapy cognitive therapy the delivery of services in coordination with the individual company service providers and third-party service individuals with related matters including their eligibility for benefits through Medicaid Medicare Social Security and private insurance. Acts as Representative Payee Designee as up-to-date case records with the status of case activity including progress notes on all other duties as RESPONSIBILITIESNone Knowledge and Skills required by the JobThe requirements listed below are representative of the knowledge skill and/or abilities required to perform the job:Education and Experience:Bachelors degree in human services or equivalent in education and experience year of related work experience and knowledge of case Licenses and Registrations:Valid drivers license and driving record that meets insurability requirements as defined by the company if applicable. Current CPR/First Aid Certification as required by state/ qualified (if required)Other Skills and Abilities:Knowledge of case managementAbility to establish working relationships with individuals servedDemonstrated competence in verbal and written communication skillsAbility to handle crisis situations with minimal supervisionLanguage Skills:Ability to read analyze and interpret general business periodicals professional journals technical procedures or governmental regulations. Ability to write reports business correspondence and procedure manuals.
