Job Description
Title : Registered Nurse - Home Health

Schedule : 8hrs

Assignment Details :
Contract Length: 8 weeks
Guaranteed Hours: 40

Requirements :
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance

Roles & Responsibilities :
- Assess and respond to the needs of patients and families in home health and hospice settings.
- Develop, document, and evaluate Plans of Care (POC) to ensure quality care and continuity.
- Educate patients and families about care, medications, and home safety tailored to their specific needs.
- Supervise and provide clinical direction to home health aides to ensure quality service.
- Monitor compliance with third-party payer requirements for assigned cases.
- Maintain effective communication with patients, families, and interdisciplinary team members.
