A registered nurse holds ultimate responsibility for direct and indirect nursing care, including assessment, planning, coordination and evaluation of care toward the goal of maintenance and restoration for optimal functioning and comfort. This includes the ability to conduct a comprehensive assessment that incorporates integration of clinical information and the ability to translate that into a plan of care for the patient/family/care team.This position requires universal skills and proficiencies which include demonstrating a comprehensive understanding of the Health Center’s mission and core values--compassion, diversity, innovation, respect, and teamwork--and the department’s immediate needs and long-term goals. In addition, the registered nurse demonstrates initiative, critical thinking, problem solving, leadership qualities, and effectively engages with the clinical team.
1. Systematically assess the health status of individuals and groups and record the related health data:
- Navigate the electronic medical record
- Collect data on health/symptoms from a patient.
2. Analyze and interpret recorded data, and make informed clinical judgments and decisions regarding the specific elements of nursing care mandated by a particular situation:
- Correlate new data and symptoms with the existing medical records
- Determine the appropriate response level within established guidelines.
3. Plan and implement nursing intervention, , prescribed medical or other therapeutic regiments mandated by the particular situation, scientific principles, recent advancements and current knowledge in the field:
- Collaborate with the patient’s provider regarding the assessment and plan as indicated. Dispense medications per orders/protocols.
- Report adverse reactions to medication and/or treatments in accordance with policy.
- Performs treatment and interventions as indicated or directed.
4. Provide and coordinate health teaching required by individuals, families and groups to maintain the optimal possible level of health:
- Educate the patient on test results, new care plans and/or other relevant health care information including the use of newly prescribed medications and/or changes in the current medication regime.
5. Evaluate outcomes of nursing interventions, and initiate change as indicated. Assess and reassess patients as needed.
6. Collaborate, communicate and cooperate as appropriate with other health care team members to ensure quality and continuity of care.
7. Serve as a patient advocate, within the limits of the law.
Department Position Summary: The primary care nurse takes a lead role in the prioritization, coordination and delivery of care by the practice team. The primary care nurse provides both direct and indirect care to patients specific to the guidelines/parameters for the department and demonstrates competency for the department specific population age and developmental stage with a focus on social determinants of health, health promotion, and prevention and management of acute and chronic health conditions. The primary care nurse reports to the Administrative Director.

2. Manages medication refills with appropriate authorization by the provider.
3. Provides thorough and concise documentation of patient’s histories, assessments, plans, and treatments. Enters messages as staff messages or telephone encounters in the computer. Enters and views patient letters in the computer. Enters exam encounters in the computer.
4. Consistently participates in the activities in the transition of care, recalls, high risk management, in basket management, and vaccines.
5. Follows protocols for management of patients with chronic/acute conditions and transitions of care.
6. Accepts constructive feedback and incorporates into plan for professional growth and development.
7. Recognizes and meets the safety needs of the departmental patient population.
8. Participates in the orientation, training, and mentoring of new employees. Provides input into the development of the orientation plan for new employees.
9. Actively participates in EBNHC programs, committees and departmental meetings.
10. Understands organizational structure of the department and unit routine and adheres to EBNHC policies
11. Responsible for continued professional growth and development.
12. Interacts with patients and families in a professional and respectful manner, which reflects the needs and concerns of the individual. Uses communication devices appropriately.
13. Promotes a sense of teamwork; maintains a positive attitude. Open to new ideas and embraces change. Provides support and direction to other members of the practice team. Promotes a culture of acceptance, support and collegiality. Shares new found knowledge willingly with team.
14. Solves problems independently, in teams, and seek consultation as appropriate.
15. Performs other departmental duties as assigned.

Knowledge, Skills and Abilities Required
EDUCATION: Graduate of an Accredited Nursing Program. Licensure in Massachusetts as an RN. BLS certification Bilingual skills preferred.
EXPERIENCE: 1 year relevant nursing experience in a community health and/or ambulatory care setting preferred.
SKILLS/ABILITIES: Demonstrated ability to communicate effectively both in writing and verbally. Familiarity with the use of a personal computer in a Windows environment preferred.
