Compassionate Case Manager: Recovery & Aftercare
The Dimock Center
The Dimock Center is seeking a Case Manager to join the clinical team. You will provide ongoing care coordination and aftercare planning for individuals receiving residential services, including advocacy, assessments, referrals, and linkage to housing, employment and services. You will collaborate with counselors and physicians, conduct weekly client meetings, document in a timely manner, and help ensure a full continuum of care through discharge planning and community referrals. #J-18808-Ljbffr The Dimock Center
