We're seeking an experienced Director of Social Work to join our skilled nursing leadership team near Hazlet, NJ . This is an excellent opportunity for a compassionate, driven professional who is passionate about resident advocacy and thrives in a collaborative environment.
Compensation: $85,000–$95,000 annually DOE
Lead the Social Work department and oversee daily operations.
Coordinate discharge planning and resident care transitions.
Support residents and families while collaborating with the interdisciplinary team.
Ensure compliance with state and federal regulations.
Current NJ LSW or LCSW required.
Skilled nursing or long-term care experience required.
Leadership experience preferred.
Strong knowledge of discharge planning, Medicare/Medicaid, and resident advocacy.
