Chenega Government Mission Solutions, LLC , a Chenega Professional Services' company, is looking for a Diabetes Pharmacist, Level II , in support of the Lawton Indian Hospital and its satellite clinics: Anadarko and Carnegie Indian Health Clinics. The Pharmacist will provide comprehensive care as the Indian Health Service-supplied facilities, equipment, and support services permit. The Pharmacist shall provide services to eligible beneficiaries of the Indian Health Service who present themselves for treatment. Responsibilities Provide emergency and routine general pharmacy services for eligible patients (children, adults, and the elderly) to include:
- Using the RPMS package and Electronic Health Record, provide all required pharmacist duties and services for the facility.
- Processing of daily prescription orders from providers, filling of prescriptions, dispensing of new prescription medications and refills, and extensive patient counseling. Assures patient understanding of medications, potential side effects, drug interactions and expected outcomes, complete all necessary documentation in pharmacy records.
- Processing, receiving and reconciling daily vendor medication orders, monitoring of the pharmacy window and pharmacy phone calls.
- Performing daily maintenance on pharmacy automation equipment, refilling of drug cassettes, prescription vials and labels.
- Management of the pharmacy call-in refill line and processing refill call-in prescriptions as indicated. Inventory management to include controlled drug perpetual inventories and "return to stock" medications.
- Politely greets and directs patients, families, and visitors utilizing public relations principles and practices.
- Applies typing proficiency in the use of such equipment as personal computers to type memoranda, correspondence etc. for the pharmacy department and providers, as requested.
- Prescription authorizations faxed and received, as needed.
- Maintains the pharmacy area in a clean and orderly manner, compliant with The Joint Commission (TJC), Safety and Infection Control standards and Environment of Care.
- Assists in department, facility and Service Unit TJC programs, as assigned. Supports and promotes the department, facility and IHS' vision and mission objectives.
- Complies with all required HIPPA responsibilities and functions.
- Maintains current licensure, attends required facility and department in-services and trainings.
- Responds correctly to all safety codes (mock or real).
- Maintains pharmacy waiting area as a safe environment, free of hazards.
- Other duties: Performs special assignments as requested that fulfill objectives of the department, facility or service unit.
- Shall provide documented evidence of annual current season influenza vaccination, unless medically contraindicated.
- Practicing intensive diabetes management as outlined in the Lawton Service Unit comprehensive pharmacy-based collaborative practice agreement
- Demonstrating proficiency in decision making using current evidence-based guidelines in diabetes management, to include the American Diabetes Association Standards of Care
- Demonstrating proficiency in continuous glucose monitors (CGMs) placement technique as well as extracting and interpreting data from CGMs in order to make medication adjustments appropriately.
- Other duties as assigned
- Graduate of accredited College of Pharmacy
- Permanent, current, full and unrestricted license to practice in the United States, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from the Board of Pharmacy specialties OR completion of a Post Graduate Year One residency
- Shall speak, understand, read and write English fluently and communicate with sufficient clarity to be understood by IHS personnel and patients.
- Shall possess sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to patients of the American Indian/Alaska Native communities.
- Shall maintain standards of personal hygiene and grooming such that his/her image as a pharmacist is compatible with the expectations of the on-site supervisor.
