Psychosocial Therapies Psycho Social Therapies is a leading mental health agency specializing in long‑term care, servicing the geriatric population residing in nursing homes and assisted living facilities throughout the state of Ohio. Our experienced and dedicated team assists residents in transitioning into long‑term care facilities and continues to work with them on any challenges they may be facing, such as anxiety, depression, and other mental health issues. Our Licensed Social Workers and Counselors deliver personalized services within the nursing home and/or assisted living facility, ensuring that each client’s unique mental health needs are met with compassion and expertise. We are seeking a qualified professional for a hybrid role that encompasses both clinical counseling and supervisory responsibilities. Responsibilities Clinical Counseling Evaluating Clients: Conduct thorough assessments to understand clients’ needs, challenges, and mental health conditions. Identify individualized support options based on these evaluations. Creating Individualized Care Plans: Develop customized care plans for each client, considering their unique circumstances and mental health requirements. Collaborating with Other Care Providers: Work closely with healthcare professionals, nursing staff, and case managers to coordinate services effectively. Facilitate seamless communication and holistic care for clients. Monitoring Clients: Regularly assess clients’ progress and adjust care plans as needed. Keep track of treatment outcomes and make necessary modifications. Maintaining Records: Document treatments, interventions, and services provided. Ensure accurate and confidential record-keeping. Providing Talk Therapy: Engage in therapeutic conversations with clients to address emotional and behavioral issues. Use evidence-based techniques to support clients in coping with challenges. Supervision Supervising Staff: Provide clinical supervision to social workers and counselors, ensuring adherence to best practices and ethical standards. Offer guidance and support to staff in managing complex cases. Training and Development: Conduct training sessions and workshops to enhance the skills and knowledge of the team. Promote professional development and continuous learning among staff. Performance Evaluation: Monitor and evaluate the performance of social workers and counselors. Provide constructive feedback and implement improvement plans as necessary. Team Coordination: Foster a collaborative and supportive team environment. Ensure effective communication and coordination among team members.
