Job Description
Title: Registered Nurse - Home Health

Schedule: 8 hours

Assignment Details:
- Contract Length: 14 weeks
- Guaranteed Hours: 40

Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- CHPN certification (preferred)
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance

Roles & Responsibilities:
- Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families regarding care, diet, medications, and home safety.
- Supervise and provide clinical direction to support staff to ensure quality and continuity of service.
- Monitor cases for compliance with requirements of third-party payers.
- Participate in interdisciplinary team meetings and other necessary meetings.
