RN Case Manager

We are seeking a compassionate and detail-oriented RN Case Manager to join a leading acute care hospital in Tallahassee, Florida . This is an excellent opportunity to support patients through comprehensive care coordination, discharge planning, and collaboration with interdisciplinary teams to ensure high-quality outcomes and appropriate transitions of care.
