Today, Greenwich House offers a diverse range of programs in health, human services, education, and the arts, serving thousands of New Yorkers each year.The OpportunityGreenwich House is seeking a dynamic, detail-oriented Caregiver Social Worker to support caregivers through core services in the Caregiver Program. Reporting to the Caregiver Program Director, this role works closely with program staff and other professionals to build community partnerships. The Caregiver Social Worker also attends staff meetings and training sessions and collaborates with organizations that serve older adults and their families.This is an exciting opportunity for someone who is passionate about people and wants to be part of a mission-driven organization making a lasting impact in New York City.ResponsibilitiesAssess caregiver stress levels and develop a mutually agreed-upon care plan to address client needs.Help clients determine eligibility for benefits and entitlements, including Medicaid and SNAP, and assist with gathering and submitting application materials when needed.Manage a caseload of caregiver clients by providing supportive counseling, information and referrals, advocacy, and ongoing case support.Assess clients' need and eligibility for respite and supplemental services.Coordinate with community agencies to advocate for clients and help ensure they receive appropriate services, benefits, and entitlements.Attend community health fairs, connect with potential caregivers, and educate attendees about available services.Increase public awareness of caregiver roles, needs, and available support services.Provide training and education to individual caregivers and to groupsMaintain accurate electronic case records and statistical reports per funder's requirements.Perform other duties as assigned.What You'll BringMaster of Social Work with good clinical skillsHave an awareness of the biological, psychological, and social aspects of aging and the impact of disabilities, illnesses, and aging on the familyAbility to arrange and negotiate service referralsSkills in establishing and sustaining interpersonal relationshipsAbility to communicate with individuals of varying socio-economic backgroundsPublic speaking skillsProven ability to work both independently and as part of a teamMust be able to work occasional evenings and weekends, 1-2 per monthProven ability to work both independently and as part of a teamBasic computer literacy – Windows, Microsoft OfficeMust be able to travel on public transportation to conduct home visits and community presentationsMust be able to climb steps to conduct home visitsPrefer bi-lingual and cultural competency in Spanish or Haitian CreoleKnowledge of community resources and public benefits eligibilityEmployment DetailsDepartment: Heights & Hills Caregiver ProgramLocation: Hybrid - 81 Willoughby Street, Brooklyn, NY 11201Schedule: 35 hours/week, Monday - Friday, 9:00 AM - 5:00 PMFLSA Classification: ExemptEmployment Type: Full-TimeReports To: Caregiver Program DirectorUnion Representation: Non-UnionSalary RangeThe expected base salary for this position is $65,000 - $68,000 annually.
