Role Responsibilities

Education Requirements

• MD or DO from a recognized medical institution.
• Completion of an accredited residency in Family Medicine.

Certifications

• Board Certification in Family Medicine.
• Possession or eligibility for an active Arizona medical license.
• BLS and ACLS certifications highly preferred.
