Physical Therapist - Home Health

Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team .

Location: Hazle Township, PA 18202
Coverage Area: Schuylkill County
Schedule: Monday-Friday, 8:00 AM-4:30 PM (40 hours/week)
Pay: $60/hour (W-2)
Job Summary:

• We are seeking a licensed Physical Therapist to provide skilled Home Health services to patients in Schuylkill County.
• The PT will evaluate patients, develop treatment plans, provide therapy, and help patients improve mobility, strength, safety, and independence.

Key Responsibilities:

• Perform patient evaluations and develop individualized treatment plans.
• Provide skilled physical therapy services in patients' homes.
• Monitor patient progress and update care plans as needed.
• Educate patients and caregivers on exercises, safety, and mobility.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians and the interdisciplinary care team.
• Meet productivity expectations of 30 points per week.

Required Qualifications

• Active Pennsylvania Physical Therapy License.
• Current CPR certification.
• Home Health experience preferred.

Preferred Skills:

• HCHB (Homecare Homebase) experience.
• Tech-savvy with strong documentation skills.
• Excellent communication and time-management skills.

On-Call/Weekend: None
Mileage Reimbursement: Yes, between patient visits
Dress Code: Business casual/professional appearance
