About This Role As a Care Coordinator RN Nurse Navigator at Cleveland Clinic, you will collaborate with Emergency Department physicians and a multidisciplinary care team to coordinate care for high-risk patients. Highlights Location: West Palm Beach, FL Facility: Weston Hospital Schedule: Full-time, Day shift What You’ll Do Collaborate with a multidisciplinary care team to develop goals and plan interventions. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Utilize assessment skills and risk assessment tools to identify patients with care needs. Conduct comprehensive clinical assessments including medical, behavioral, and social needs. Inform and work with patients and families regarding care coordination. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across settings. Identify barriers, facilitate solutions, and connect to community resources. Ensure the chart contains necessary information and is updated as needed. Requirements Education: Graduate from an accredited school of Professional Nursing (Diploma, ADN, BSN program) required Experience: 3-5 years of nursing experience required Licenses & Certifications: Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) certification required ACLS certification preferred About Cleveland Clinic Cleveland Clinic is a leading healthcare provider committed to delivering exceptional patient care.
