Role Description The Physical Therapist will provide on-site patient care at our Granite City, IL clinic as part of a contract role. Day-to-day responsibilities include evaluating patients, developing and implementing individualized treatment plans, and delivering evidence-based interventions focused on mobility, strength, balance, pain management, and functional independence. The Physical Therapist will document assessments and progress, collaborate with referring providers and interdisciplinary team members, and educate patients and caregivers on home programs, safety, and prevention strategies. Additional duties may include participating in fall prevention initiatives, supporting neurological and orthopedic rehabilitation programs, and contributing to community education activities as needed. Qualifications Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and adjust individualized treatment plans. Demonstrated skills in Patient Care and Home Care, including education on home exercise programs and safety strategies. Working knowledge of general Medicine principles relevant to musculoskeletal, neurological, and orthopedic conditions. Current license (or eligibility for licensure) as a Physical Therapist in the state of Illinois. Degree in Physical Therapy from an accredited program (Doctor of Physical Therapy preferred). Experience in outpatient or adult rehabilitation settings, including balance, fall prevention, and post-surgical recovery, is beneficial. Ability to provide compassionate, culturally sensitive, and patient-centered care to diverse populations. Strong interpersonal, communication, and documentation skills, with the ability to collaborate effectively in a multidisciplinary team.
