Population Health Program Manager

Population Health Program Manager---------------------------------------------------------------The Population Health Program Manager is accountable for the success of assigned population health programs and initiatives serving Medicaid and Medicare members. This role translates population health strategy into operational execution by leading the design, implementation, and evaluation of internal/external programs, services, and processes that address the needs of defined populations and advance organizational goals related to health outcomes, equity, quality, cost, utilization, and member experience.This is a hybrid position with the expectation to be in office 1-2x per month.Estimated Hiring Range:$102,330.00 - $125,070.00Bonus Target:Bonus - SIP Target, 5% AnnualCurrent CareOregon Employees: Please use the internal Workday site to submit an application for this job.---------------------------------------------------------------Essential ResponsibilitiesLead the design, implementation, and ongoing management of evidence-based population health programs, interventions, and processes leveraging internal resources and external partners to meet defined objectives.Provide matrix leadership to coordinate population health initiatives across internal departments and teams, aligning scope, timelines, roles, and deliverables.Define program success measures and monitor performance using population health, quality, utilization, and experience data; evaluate program effectiveness; identify gaps; and lead continuous improvement activities in partnership with analytics and operational teams.Develop and implement strategies to engage members, providers, and community partners to support program adoption, participation, and adherence, including development of operational workflows and communication plans.Prepare and present routine and ad hoc status updates, performance summaries, and recommendations to leadership and governance forums.Ensure programs align with organizational priorities, regulatory requirements, and best practices.Identify and support partnerships across physical health, behavioral health, oral health, social health, and community organizations.Collaborate across organizational departments and lines of business to align program priorities and improvement actions.Provide technical assistance and training to internal teams and external partners.Maintain awareness of applicable regulations, accreditation requirements, and emerging best practices in population health management; assess impact and recommend updates to programs, workflows, and measures.Represent the organization on program-specific local, state, or national workgroups and committees.Develop and manage project plans, including timelines, milestones, and KPIs.Apply quality improvement and project management tools to improve program performance.Organizational ResponsibilitiesPerform work in alignment with the organization’s mission, vision and values.Support the organization’s commitment to equity, diversity and inclusion by fostering a culture of open mindedness, cultural awareness, compassion and respect for all individuals.Strive to meet annual business goals in support of the organization’s strategic goals.Adhere to the organization’s policies, procedures and other relevant compliance needs.Perform other duties as needed.Experience and/or EducationRequiredMinimum 6 years of experience in population health, public health, health care management, quality improvement, or a related fieldAt least 2 years in project or program managementDemonstrated matrix leadership (no direct reports required), including leading projects/work groups, influencing stakeholders, and driving accountability for deliverablesExperience leading cross-functional teams and managing complex initiativesExperience achieving impact through program or project managementExperience using population health data to drive decisions and actionPreferredMaster’s degree in public health, healthcare administration, business, or related fieldExperience in managed care, CCOs, or population health/quality programsExperience implementing innovative or pilot programs and scaling effective practicesKnowledge, Skills and Abilities RequiredKnowledgeStrong expertise in Medicaid and Medicare programs, regulations, and managed care operationsExpertise in population health strategies, value-based care, and quality improvementAdvanced knowledge in quality improvement methodologies and health care performance metrics (HEDIS, NCQA, CMS Star Rating, CCO Metrics)Expertise in program design, implementation, and monitoringKnowledge of healthcare delivery systems, care management operations, social determinants of health, and community resourcesSkills and AbilitiesProven ability to lead through influence, facilitate cross-functional decision-making, and drive accountability for deliverablesProficiency in interpreting and analyzing healthcare data from dashboards and reports (e.g., Tableau, Power BI, Excel, or similar tools)Advanced Excel functions for manipulating, analyzing, and visualizing population and claims data to inform data-driven decision-making and actionable recommendationsExperience in program evaluation, cost analysis, and identifying trends in healthcare utilizationStrong leadership and team management skills with the ability to oversee large-scale initiativesExcellent project management skills, including experience with Agile methodologies and performance trackingProven ability to build and maintain relationships with healthcare providers, community organizations, and government agenciesEffective communication and executive presentation skills, with the ability to translate complex data into actionable insightsAbility to manage competing priorities and deliver results in a fast-paced, highly collaborative environmentStrong analytical and problem-solving skills; ability to synthesize complex data into clear insights and actionable recommendationsExperience with quality improvement methodologies and other process optimization frameworksAbility to lead independently in a fast-paced environment, maintaining alignment between complex programs and organizational strategyExperience developing policies/procedures, facilitating governance forums, or managing vendors/contractsAbility to work effectively with diverse individuals and groupsAbility to learn, focus, understand, and evaluate information and determine appropriate actionsAbility to accept direction and feedback, as well as tolerate and manage stressAbility to see, read, and perform repetitive finger and wrist movement for at least 6 hours/dayAbility to hear and speak clearly for at least 3-6 hours/dayWorking ConditionsWork Environment(s): Indoor/Office Community Facilities/Security Outdoor ExposureMember/Patient Facing: No Telephonic In PersonHazards: May include, but not limited to, physical and ergonomic hazards (Indoor/Office)OREquipment: General office equipmentTravel:May include occasional required or optional travel outside of the workplace;
