Key Responsibilities\nIn this role, your responsibilities will include:

- Maintaining accurate and timely clinical documentation that adheres to HIPAA regulations, ensuring confidentiality and high standards of care. \n
- Working alongside a multidisciplinary treatment team to coordinate care, bringing together insights from psychiatrists, psychologists, and other professionals to improve patient outcomes. \n
- Engaging in regular case consultations and peer review meetings, fostering a culture of learning and improvement within the organization. \n
- Supporting patients and their families through education on mental health issues, available treatment options, and resources within the community to empower their decision-making process. \n

The patients you will serve at our hospice center include individuals facing terminal illnesses and their families. Qualifications\n- \n
- A Master's degree in Social Work from an accredited institution, demonstrating your academic foundation in the field. \n
