Key Responsibilities\n

• Conduct comprehensive clinical assessments to determine the specific needs of patients, developing personalized treatment plans that are evidence-based and focused on individual goals.

• Implement various therapeutic interventions including but not limited to cognitive-behavioral therapy (CBT) and mindfulness practices to support patients dealing with grief, anxiety, and depression.

• Maintain accurate and up-to-date clinical documentation in compliance with HIPAA and other regulatory standards, ensuring confidentiality and professional accountability.

• Work collaboratively with a multidisciplinary team of healthcare professionals, including nurses, physicians, and fellow social workers, to provide holistic care for clients and their families.

• Engage in regular case consultations and peer review meetings to refine treatment methodologies, benefiting both professional development and quality of care.

• Educate patients and families about available resources, coping strategies, and potential treatment avenues, empowering them to make informed decisions regarding their care.

Qualifications\n
