Key Responsibilities\n

• Conduct initial and ongoing comprehensive assessments of patients to evaluate their physical, emotional, and social needs.

• Create and implement tailored treatment plans that address the unique challenges faced by each patient.

• Utilize evidence-based practices to deliver effective therapeutic interventions that promote positive outcomes and support patients' coping mechanisms.

• Maintain thorough, accurate, and confidential patient records in compliance with HIPAA regulations and organizational policies.

• Collaborate with an interdisciplinary team to ensure a holistic approach to patient care, discussing updates and adjustments to care plans as needed.

• Participate in regular training sessions and meetings aimed at professional development and staying informed of the latest advancements in hospice care.

Patient Population / Client Focus\n

Your work will primarily focus on individuals receiving hospice care, many of whom are facing terminal stages of illness. Supporting their families through the emotional journey of grief is equally important. You will utilize a trauma-informed approach to understand their unique experiences, fostering a safe space for open communication and emotional exploration.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active licensure as a Licensed Independent Clinical Social Worker (LICSW) in Massachusetts.

• Demonstrated ability to communicate effectively with patients, families, and other healthcare professionals.

• Strong problem-solving skills and the capacity to thrive in a collaborative team environment.
