Key Responsibilities\n

• Conduct individual and group therapy sessions via telehealth platforms, adapting therapeutic techniques to meet diverse client needs.

• Develop and implement individualized treatment plans that address each patient's unique goals and challenges, ensuring alignment with best practices in hospice and palliative care.

• Collaborate closely with a multidisciplinary team, including physicians, nurses, and administrative staff, to provide coordinated care and optimize patient outcomes.

• Maintain accurate, confidential patient records utilizing Electronic Health Record (EHR) systems, ensuring compliance with legal and ethical standards.

• Provide psychoeducation to patients and families, helping them understand emotional responses to grief and loss and equipping them with coping strategies.

• Facilitate crisis intervention as needed, ensuring immediate and appropriate responses to patient distress.

Patient Population / Client Focus\n

The primary focus of this role is to support patients facing end-of-life issues and their families. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.
