Key Responsibilities\n

• Conduct comprehensive psychosocial assessments virtually, effectively evaluating clients' unique strengths and challenges.

• Deliver evidence-based therapeutic interventions and counseling services to individuals, families, and groups, ensuring a tailored approach for each client.

• Collaborate closely with a multidisciplinary team, including healthcare providers, to devise integrated care strategies that address clients' physical and mental health needs.

• Advocate for clients' rights, assisting them in navigating access to essential community resources and support services.

• Maintain detailed documentation of client interactions, treatment progress, and outcomes through our electronic health record system.

• Engage actively in team meetings and case conferences, sharing insights and updates to enhance the quality of care provided to clients.

Patient Population / Client Focus\n

At our Hospice Center, you will work with a diverse group of clients, including those facing end-of-life issues, chronic illnesses, and their families. You will provide empathetic support to help them cope with the emotional and psychological challenges associated with these experiences. Your expertise will play a critical role in facilitating healing and managing grief, ultimately aiding clients and families in achieving greater peace during difficult times.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington state.

• At least two years of post-licensure experience in clinical social work settings, particularly with vulnerable populations.

Preferred Experience\n
