Pediatric experience is a plus. Provides a complete physical assessment and history of current and previous illness(es).Regularly re-evaluates patient nursing needs.Initiates the plan of care and makes necessary revisions as patient status and needs change.Develops the plan of care with patient's physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. Includes the patient and family in the planning process.Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physicianCounsels the patient and family in meeting nursing and related needs.Provides health care instructions to the patient/family as appropriate per assessment and plan.Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patientKnowledge of high-tech nursing skills as applicable which is not limited to indwelling/straight catheter insertion/removal, nephrostomy care, ostomy care, plural and abdominal catheter drainage care, infusion services, g-tubes, and negative pressure wound therapy.Working knowledge of all infection control procedures and safety precautions.Knowledge of the nurse's scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process.Observe ethical and legal guidelines for safeguarding confidentiality of patients.Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical assessment, response to therapy and any updated physician's orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions.Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes and submits all documentation in accordance with policy and procedures.Communicates with the physician regarding the patient's needs and reports and changes in the patient's condition; obtains/receives physician's orders as required.Communicates with other health related persons to coordinate care plan.Participates in on-call duties as defined by the on-call policy.Ensures that arrangements for equipment and other necessary supplies and services are available.Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation.Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days.Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists.Qualifications:Bachelor's degree of Nursing from an accredited college/universityCurrent active and unrestricted nursing licensePediatric care experience in the acute and/or home care environmentValid driver's licenseCurrent certification in Basic Life Support for Health Care Professionals.Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting.Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.Basic knowledge of computer operating systems and software applications to communicate and document care.Job Type: Fee-for-Service, Paid per VisitWe are excited to welcome passionate and dedicated individuals to join our team at Personal Touch Home Care.
