Behavioral Health Case Manager (Southwest)

Job Overview We are seeking a dedicated and compassionate Case Manager to join our BH team at Merrionette Park. The role involves providing comprehensive services tailored to a diverse client population, helping enhance their potential and promote optimal functioning within the community. Salary The average starting salary for this position will fall in the range of $42,000 to $47,000 annually, based on skill and experience level. Responsibilities Conduct thorough assessment of clients. Evaluate problems and service needs. Develop a written client service plan specifying goals to be achieved and services to be pursued. Supplement service plans to meet client needs. Provide, as appropriate, supportive help, advocacy and case management to individuals and families coping with environmental and interpersonal relationship issues. Work collaboratively with community resources. Develop a knowledge of resources and help clients utilize them. Work within contract, grant, and program requirements including meeting timelines and reporting requirements. Engage clients in settings that may present a challenge for the client for the purpose of learning, practicing, and developing healthy behaviors and coping skills. Reassess and monitor client situation and adequacy of service plan. Collaborate and participate with other program staff in the planning and implementation of service plans. Subsequent to consultation with supervisor, handle emergency and crisis situations within target population. With supervisory approval, authorize limited amounts of financial assistance to own clients. May work with community groups. Work effectively with diverse staff and service population. Other duties as assigned. Qualifications Bachelor's Degree in Psychology, Social Work, or related field from an accredited college or university. One year of case management experience in a social service or healthcare setting. Certified Alcohol and Drug Counselor certification preferred. Bilingual in Spanish preferred. Crisis stabilization experience preferred. Ability to work evenings as needed. Additional Requirements Satisfactorily pass all relevant screening and criminal background checks. Travel / Transportation Requirements Driving for work required with transportation of clients preferred in a personally owned vehicle. Physical Demands While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal. Must be able to lift at least 30 lbs. Benefits Medical Coverage, Dental Coverage, and Vision Coverage.
