RN Case Manager, Home Health & Hospice
Join a respected home health and hospice team thats passionate about delivering patient-centered care across both home health and hospice settings. This is a W2, direct-hire position with our client, a well-established provider serving the Lewiston community.
Important Requirement:
- Candidates must live in Lewiston or within 30 minutes of Lewiston to be considered for this position.

What Youll Do:
- Conduct thorough patient assessments and develop individualized care plans.
- Coordinate with interdisciplinary teams to ensure seamless care.
- Provide end-of-life care and symptom management for hospice patients.
- Educate patients and families on managing health conditions at home or during hospice.
- Maintain accurate and timely documentation for both home health and hospice services.
Position Details:
- Location: Lewiston, ID
- Schedule: Full-time, day shift
- Compensation: Competitive salary based on experience
- Bonuses: Relocation and sign-on bonuses considered (case-by-case)
- Employment Type: W2, direct hire with our client
What Youll Bring:
- Active RN license in Idaho (or compact state)
- At least 1 year of experience in home health and/or hospice
- Excellent communication and organizational skills
- Commitment to patient-centered care
- Residency in Lewiston or within 30 minutes of Lewiston
