Physical Therapist PRN

Role Overview: A physical therapist assesses, plans, and implements rehabilitative programs to improve mobility, reduce pain, increase strength, and prevent deformity in patients with injuries or diseases. Services are delivered across the continuum in a cost-effective, quality-focused, patient-centered manner, ensuring compliance with regulatory standards. Key Responsibilities Patient Assessment: Collaborate with physicians to develop and evaluate treatment plans, goals, and discharge strategies, involving patients and families in care planning. Treatment Planning and Care: Execute treatment plans, select appropriate activities, adjust goals with patient input, provide education, and coordinate follow-up and referrals. Act as a resource for staff, families, and physicians regarding rehabilitation services. Documentation: Record evaluations, treatment sessions, family education, discharge details, and other required documentation per protocols. Professional Conduct: Uphold ethical standards, maintain confidentiality, punctuality, and adhere to dress code. Communicate effectively, participate in meetings, and utilize resources safely and efficiently. Customer Service & Professional Development: Strive for excellent service, foster teamwork, apply AIDET principles, participate in ongoing education, and complete mandatory training and licensure requirements. Qualifications & Skills Masters or Doctorate in Physical Therapy (preferred) Licensed PT and BLS certification required Minimum one year of experience in rehab, LTACH, acute care, or SNF settings Effective communication, customer service orientation, teamwork skills, and basic computer proficiency Additional duties include performing other assignments and complying with all organizational policies and standards. #J-18808-Ljbffr Vetted Solutions
