Description:Licensed Practical Nurse (LPN) – Home HealthPosition Summary

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes around Colorado Springs . The LPN works in coordination with the RN/Nursing Supervisor, physicians, caregivers, and other members of the care team to provide safe, high-quality care according to each patient’s established Plan of Care.

This position is ideal for an LPN who enjoys one-on-one patient care, building relationships with families, and working independently in a home health environment while having support from an experienced clinical team.

Responsibilities

• Provide skilled nursing care in accordance with the patient’s physician-ordered Plan of Care and agency policies.
• Perform nursing treatments and procedures within LPN scope of practice.
• Monitor and document the patient’s condition, including vital signs, symptoms, response to treatment, and changes in physical or emotional status.
• Report significant changes in condition promptly to the supervising RN and/or physician.
• Administer medications and treatments as ordered.
• Maintain knowledge of current medications, potential side effects, and medication precautions.
• Provide education to patients and caregivers regarding medications, treatments, disease management, safety, and ongoing care needs.
• Assist with preventive and rehabilitative nursing interventions as directed by the Plan of Care.
• Complete accurate and timely clinical documentation for all services provided.
• Communicate regularly with the RN/Nursing Supervisor regarding patient progress, concerns, and changes in care needs.
• Participate in case conferences and care coordination as requested.
• Coordinate with other members of the healthcare team to support continuity of care.
• Participate in discharge planning and changes to the Plan of Care under the direction of the supervising RN.
• Follow all applicable agency policies and federal and state healthcare requirements.
• Participate in staff meetings, training, and continuing education.
• Provide coverage for other nursing staff when available and requested.
• Maintain patient confidentiality and comply with HIPAA requirements.

Patient Care Duties

The LPN may provide services including, but not limited to:

• Medication administration
• Vital sign monitoring
• Ongoing nursing assessments within LPN scope
• Respiratory treatments and monitoring
• Enteral feeding and G-tube care
• Seizure monitoring and seizure precautions
• Diabetic care and blood glucose monitoring
• Wound and skin care
• Catheter and bowel/bladder care
• Infection prevention
• Patient and caregiver education
• Monitoring response to medications and treatments
• Documentation of changes in patient condition
• Other skilled nursing services ordered by the physician and permitted within the LPN's scope of practice

Care Coordination

The LPN works closely with the supervising RN and interdisciplinary care team to:

• Communicate changes in patient condition.
• Assist with implementation of the established nursing Plan of Care.
• Identify potential changes in nursing needs and report them to the supervising RN.
• Communicate concerns from patients and caregivers.
• Help ensure physician orders and treatment instructions are followed.
• Participate in care conferences and ongoing evaluation of patient progress.

Requirements:Job Requirements

• Must be a graduate of an accredited practical nursing program.
• Must hold a current and active Colorado Licensed Practical Nurse (LPN) license .
• Current CPR/BLS certification required.
• Previous home health, pediatric, long-term care, hospital, or private-duty nursing experience preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently while following an established Plan of Care and clinical supervision.
• Strong organization and time-management skills.
• Ability to complete clinical documentation accurately and on time.
• Must successfully complete required background screening.
• Valid driver's license, reliable transportation, and ability to travel to patient homes are required.

Physical Requirements

This position requires the ability to:

• Travel to and safely access patient residences.
• Walk, sit, stand, bend, stoop, and perform other movements associated with providing direct patient care.
• Assist with positioning, transferring, and caring for patients as appropriate.
• Have sufficient vision, hearing, communication ability, and physical dexterity to safely perform nursing duties.
• Lift and assist patients as required by the individual care assignment.

• Blood pressure cuff
• Thermometer
• Pulse oximeter
• Glucometer
• Stethoscope
• Enteral feeding equipment
• Respiratory equipment
• Other medical equipment required by the patient's Plan of Care
