Home Health Physical Therapist
This is a Home Health Physical Therapist opportunity in Seaside, OR and surrounding areas. The facility is a Home Health setting operating under Providence Portland Home Health North Coast. The system does a good job grouping clients to minimize drive and windshield time, with an occasional maximum radius of roughly 60 miles (6090 minute total drive time) per day.
Key Responsibilities
- May visit up to 6 patients per day; 30 productivity points standard (1:1 ratio).
- Complete Oasis assessments as required.
- Utilize EPIC Remote Client for Home Health (on laptop) and Rover (phone app) for charting.
- Provide skilled home health physical therapy services to patients across the assigned service area.
Required Skills
- 1 year of Skilled Home Health Experience required.
- Proficiency with Oasis documentation (required).
- Proficiency with EPIC Remote Client for Home Health and Rover (phone app).
- Must have own transportation.
Education Requirements
- PT License required.
- BCLS (Basic Cardiac Life Support) certification required.
Required Experience
- Minimum 1 year of Skilled Home Health Experience required.
Work Environment / Physical Requirements
- Type of Facility: Home Health.
- Attire / Scrub Color: Business casual, no jeans; scrubs allowed but not required (no specific color).
- Parking Cost: Free.
- Mileage Reimbursement: IRS Standard.
- Traveler is expected to have their own transportation.
- Occasional maximum radius of roughly 60 miles (6090 minute total drive time) per day.
- Shift cannot accommodate 4x10s or 3x12s.

- Facility requires primary series COVID vaccination in addition to one booster.
- Weekend requirement: No.
- On-call required: No.
- Float between like units: No.
- Block schedule: Not available.
- Locals accepted: Yes. Considered "local" within 5075 mile radius; locals accepted at the same rate.
- Pending License Accepted: Yes.
- RTO Restrictions: Confirm with hiring manager.
- Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks).
- First-Timers Accepted: Yes.
