Navigator - Clinics & Physician Support - Total Heart Canter - FT - MRH

Location: Hollywood, Florida Summary: The Navigator is responsible for guiding patients through the complexities of the healthcare system. Working collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients, the Navigator ensures that safety, best practice, and high quality standards of care are maintained across the continuum. This role provides support and resources to patients, including connecting patients to available services, navigates insurance authorizations, coordinates care between providers, and works with healthcare providers to ensure that patients are receiving the best possible care. Responsibilities: Coordinates a wide range of activities and education to support the patient population by utilizing a variety of communication methods including but not limited to telehealth, face-to-face, text, telephone, or e-mail. Works closely with physicians and the multidisciplinary care management teams. Facilitates care coordination, and documents efforts. Utilizes the appropriate reports to prioritize outreach to ensure care coordination. May assist with obtaining authorizations and scheduling where appropriate. Extracts data from medical record sources to gather information and perform outreach. Communicates details to the appropriate team, providers, and/or supervisors.
