On-Call Case Manager — In-Home Family Services

Northeast Family Services

Northeast Family Services is seeking a dedicated Case Manager to support families in New Hampshire. The role reports to the Clinical Supervisor and acts as a liaison among the treatment team, clients, families, and DCYF, coordinating in-home services and ensuring high-quality documentation. You will manage caseloads, develop measurable treatment plans with families, and participate in court-related reporting, providing education and supervision in home and community settings. #J-18808-Ljbffr Northeast Family Services
