Primary Functions:Provide outreach services to children and familiesConsult with team members in the development of individualized plans and multidisciplinary services for assigned clientsMaintain client records completely and accurately to include IFSP's with full assessment of client functioning and goals, Documentation of client progress in the program, records of required contact with collaterals and family members, Documentation of arena assessments and documentation of DPH forms and regulations regarding intake and discharge of clientsComplete billing documentation according to schedule established by the companyAdhere to professional standards and guidelines according to their respective disciplinesKeep supervisor informed of progress of all clientsMeet direct service requirements based on part time or full time status.
