Care Management Coordinator

$19.97 - $30.54 per hour

Description The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance. Required Qualifications 2 years administrative experience; healthcare setting preferred or demonstrated knowledge of healthcare terminology. Preferred Qualifications Associate's Degree in Business, Healthcare, or related field. Additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
