Responsibilities:

• Provide direct patient care to a diverse range of residents, including those with complex medical conditions and rehabilitation needs

• Assist with medication administration, vital signs, and other clinical tasks as needed

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Document patient information accurately and timely in accordance with regulatory requirements

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Demonstrate a commitment to safety, dignity, and respect for all residents and colleagues

Requirements:

• Current LPN licensure in Pennsylvania

• High school diploma or equivalent required

• Previous experience in a long-term care or rehabilitation setting preferred

• Basic Life Support (BLS) certification required

• Ability to lift up to 50 pounds and stand for extended periods

• Strong communication and interpersonal skills
