Registered Nurse, OR Licenses: RN(Compact Accepted)(Copy needed) Certifications: BLS-AHA(Copy needed)

ACLS-AHA(Copy needed) SSN Required DOB Required Work History: Should not be more than 29 days of gap in References: 1 Reference in the last 12 months Ref within 12 mos for job worked within 5 yrs Skills checklist within 12 mos RTO Upon Submission perm address outside 75 miles work Mix day shifts for 40 hr week ( cannot do solely 10s or solely 12s)
