Work Environment

This position will require you to work onsite directly in patient homes, allowing you to offer personalized treatment plans that cater to the individual needs of each patient. Key Responsibilities

• Conduct thorough evaluations of patients' conditions through initial consultations, including analyzing their physical symptoms and movement capabilities.
• Develop and implement effective treatment plans tailored to each patient's unique needs, goals, and anticipated outcomes.
• Educate patients on the proper techniques for therapeutic exercises and activities that will facilitate their recovery.
• Utilize evidence-based modalities such as manual therapy, therapeutic exercises, and equipment assistance to promote healing and restore functionality.
• Document and maintain accurate patient records detailing progress, treatment sessions, and long-term goals.
• Advise patients and family members about necessary in-home treatment strategies and exercises to ensure ongoing health management.
• Collaborate with a multidisciplinary team to optimize patient care and share insights regarding treatment efficacy.

Patient Population / Client Focus

Your patients will include a diverse array of individuals ranging from children to older adults, each with unique circumstances and diagnoses. You will provide care for conditions such as sports injuries, chronic pain, post-operative recovery, and neurological disorders. Your ability to adapt your approach to address the specific challenges faced by each patient will be essential in achieving successful outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current state license to practice physical therapy in Virginia.
• Strong written and verbal communication skills to effectively interact with patients and healthcare providers.
• Proficient in using electronic medical records (EMR) systems and standard computer software for documentation and reporting.
• A high degree of adaptability, with the ability to manage physical demands and work flexibly in various home settings.

Preferred Experience

While a minimum of 2 years' experience in a clinical setting is required, candidates with experience in home health care are especially encouraged to apply. Experience working with diverse patient populations will be beneficial in navigating the varied challenges encountered in this role.

Compensation and Benefits
