Work Environment

This position is fully onsite, providing home-based therapy services within Arlington and surrounding areas. You will engage directly with patients in their home environments, which allows for a more personalized assessment and treatment approach. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, including assessing strength, mobility, and functional ability to develop effective treatment plans.
• Design and implement comprehensive treatment programs tailored to individual patient needs, focusing on rehabilitation and recovery.
• Educate patients and their families on therapeutic exercises, pain management strategies, and lifestyle changes to support health and wellness.
• Continuously monitor patient progress through regular re-evaluations and adjust treatment plans as necessary to achieve optimal outcomes.
• Collaborate with interdisciplinary teams, including physicians and healthcare providers, to ensure seamless continuity of care.
• Document all patient interactions, assessments, treatment plans, and progress notes meticulously to comply with legal and ethical standards.
• Participate in community outreach programs to promote physical health and educate the public about injury prevention and wellness.

Patient Population / Client Focus

The primary focus of this role will be on patients with various functional limitations and disabilities, including post-surgical rehabilitation, chronic pain management, and age-related mobility issues. You will have the opportunity to work with a diverse demographic, ensuring that care is equitable and culturally sensitive to meet the specific needs of each patient.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Current state licensure in Physical Therapy and BLS certification are required.
• Strong written and verbal communication skills, alongside proficiency in clinical documentation and electronic health records.
• Ability to work independently while managing a positive and productive caseload.

Preferred Experience

• A minimum of two years of experience as a practicing Physical Therapist, specifically in a home health or outpatient physical therapy setting.
• Experience with geriatric patient populations and knowledge of age-specific considerations for therapy.

Compensation and Benefits
