Requirements:

• Minimum 2 years acute care Case Management experience
• Acute care hospital experience required
• Must be open to floating between like units based on need
• Weekend requirement: Every Other Weekend
• Experience with Epic charting preferred
• No call required

Preferred:

• Strong discharge planning and care coordination background
• Ability to adapt to department needs

Notes:

• First-time travelers accepted with Clinical approval
• Locals accepted
• Parking details provided during orientation
• Facility offers housing assistance for travelers if needed
