Patient Engagement Specialist About Our Client Our Client is a venture-backed healthcare technology company on a mission to transform cardiovascular care. Its platform helps large provider groups and health systems succeed in value-based care arrangements by improving patient outcomes and reducing the cost of care through stronger coordination, analytics, and disease management. Our Client currently supports multiple value-based care contracts across more than ten states and has significant growth plans for 2026 and beyond. Although its initial focus is cardiology, the organization is building a scalable operating system for managing complex conditions across the healthcare spectrum. Position Overview Our Client is seeking a Patient Engagement Specialist to join its growing care team. This individual will be responsible for new patient enrollment and will play a critical role in supporting organizational growth. The Patient Engagement Specialist will work within partner cardiology practices and help identify, educate, and enroll eligible patients into the client’s care program. The position involves collaborating with physicians and practice staff, communicating with prospective patients, explaining available services, answering questions, gathering information, providing education, and scheduling initial appointments with the virtual care team. The successful candidate must be comfortable serving as the primary on-site representative of Our Client at partner locations. This person must be capable of working independently and making sound day-to-day decisions that reflect the organization’s values, service standards, and commitment to patient care. Key Responsibilities Maintain a daily on-site presence at assigned partner cardiology practices. Support additional partner clinics remotely, as needed. Pre-screen patients for program eligibility based on established criteria and patient risk profiles. Notify practice providers of prospective patient enrollments. Verify patient eligibility and gather feedback from practice providers regarding enrollment potential. Conduct patient enrollment and onboarding sessions following referrals from partner practitioners. Educate prospective patients about the care program, available services, and potential benefits. Answer patient questions and address concerns regarding enrollment and program participation. Maintain accurate documentation within Our Client’s care-management system and the partner cardiology practice’s electronic medical record system. Facilitate patient assignment and transfer to the virtual care team for continued services. Coordinate with partner cardiology practices to provide visibility into each patient’s care journey. Communicate regularly with practice staff regarding patient admissions, readmissions, and emergency department visits. Manage inbound calls from patients regarding enrollment and ongoing participation in the care program. Develop and implement strategies to increase appropriate patient enrollment. Deliver educational presentations and program updates to physicians, administrators, and practice staff. Communicate regularly with members of the interdisciplinary care team to share information, coordinate responsibilities, and support the enrollment process. Follow each partner practice’s clinical guidelines, protocols, and operational standards. Work closely with the clinic’s Practice Administrator to ensure alignment with clinic operations and standards of care. Participate in special projects and organizational initiatives as assigned. Required Qualifications At least three years of experience working independently in a healthcare, clinical, or physician-practice environment. Experience interacting directly with patients, physicians, providers, and practice staff. Demonstrated ability to work autonomously in a clinical setting. Ability to manage competing priorities and make sound decisions without direct on-site supervision. Proven ability to represent an organization professionally with physicians, practice leadership, patients, and families. Strong written and verbal communication skills. Ability to clearly explain programs, workflows, services, and next steps to clinical and non-clinical audiences. Strong professional presence, reliability, accountability, and follow-through. Ability to remain organized, prepared, and punctual in a fast-paced environment. Comfort working within a growing organization where processes and priorities may continue to evolve. Ability to quickly learn and effectively use multiple technology platforms, including electronic health records, enrollment systems, and care-management tools. Experience handling sensitive patient information with discretion and sound judgment. Ability to proactively identify issues, elevate concerns appropriately, and recommend potential solutions. Strong attention to detail, particularly when documenting patient information, verifying eligibility, and coordinating care-team handoffs. Healthcare certification such as CMA, CHW, CPhT, RHIT, or equivalent hands-on healthcare experience. Preferred Qualifications Previous experience working within a specialty medical practice. Cardiology experience is strongly preferred. Experience in patient enrollment, patient engagement, care coordination, population health, or value-based care. Experience educating patients about healthcare programs or services. Comfort leading small-group training sessions or educational presentations for physicians and clinic staff. Ideal Candidate The ideal candidate is confident, patient-focused, highly organized, and comfortable working independently. This individual understands how to build trust with patients and clinical teams while maintaining the accuracy, responsiveness, and professionalism required in a healthcare setting. This position is well suited for someone who enjoys interacting with patients, developing strong relationships with medical practices, solving problems, and helping eligible patients gain access to meaningful care and support.
