Responsibilities

• Provide direct patient care to residents in a long-term care setting under the supervision of a registered nurse and charge nurse.
• Administer prescribed medications and document administration accurately in the resident’s chart.
• Monitor vital signs, perform routine assessments, and report changes in resident condition promptly.
• Assist residents with activities of daily living, promote safety and comfort, and adhere to individualized care plans.
• Maintain accurate documentation, follow infection control protocols, and participate in shift handoffs and interdisciplinary communications.

Requirements

• Active Licensed Practical Nurse license to practice in New Hampshire (or ability to obtain promptly).
• Relevant clinical experience in long-term care or geriatric settings is preferred.
• Ability to work evenings (approximately 2:30 PM–11:00 PM) and accommodate shift needs.
• Current Basic Life Support (BLS) certification or ability to obtain prior to start.
• Local resident or commuting distance to Lancaster, NH; local assignment only.
• Ability to perform the physical tasks customary to the role (standing, lifting, mobility assistance, and patient care activities).
