Home-Based LPN Assessor for Elderly & Disabled Waiver
PruittHealth - Santa Rosa
PruittHealth in Savannah, GA is seeking an LPN Assessment Nurse to conduct initial and annual home assessments for elderly and disabled waiver clients. You will verify ADLs/IADLs, review medications, and ensure services support community living.
You will coordinate with Medical Director and Home First team, maintain documentation in Harmony, and comply with all policies and regulations while providing compassionate, patient-centered care.
