Key Responsibilities\n

• Conduct comprehensive psychosocial assessments utilizing interviews, observational skills, and review of patient medical histories to evaluate emotional, social, and environmental factors that may impact recovery.

• Provide ongoing counseling and emotional support to patients and their families, assisting them in navigating adjustment challenges and enhancing overall emotional wellbeing.

• Collaborate closely with the rehabilitation team to craft individualized treatment plans that effectively integrate patients' psychosocial needs into their care strategies.

• Facilitate access to essential community resources, such as support groups, vocational rehabilitation, and housing assistance, ensuring patients receive comprehensive care.

• Educate patients and families about available community resources and post-rehabilitation services, empowering them to take an active role in their recovery.

• Engage in discharge planning and transitional care coordination, ensuring smooth transfers to home or appropriate care settings following therapy.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Kansas.

Compensation and Benefits\n

The position offers a competitive salary range of $75,000 to $100,000 annually, commensurate with experience and qualifications. In addition to a robust compensation package, employees are eligible for a sign-on bonus.
