In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Okanagan, WA

Job Description Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. These visits occur in members’ homes, allowing clinicians to build trusted relationships and deliver comprehensive, personalized health assessments within the comfort of the client’s environment. In this role, you will: Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes. Meet millions of people nationwide by providing connections to the right care for their unique needs. Work with Signify Health as a clinician colleague and fulfill the role’s requirements. Be flexible to travel locally and within licensed states, as needed. Conduct virtual visits as required. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE). A Note on Our Work Environment As an in‑home clinician, your primary workplace is the private residence of health‑plan members. Candidates must be able to comfortably and safely perform duties in environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Required & Preferred Qualifications (NPs and PAs) Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Ability to cover the Okanagan and surrounding area. Per Diem Employee Compensation and Benefits Anticipated Weekly Hours: 12 Time Type: Part time Pay Range: $46.03 – $99.14 per hour. This range represents the base hourly rate for the position; the actual base offer will depend on experience, education, geography, and other factors. Hourly base rate plus additional incentives (e.g., per visit completed). Malpractice insurance coverage during Signify Health‑related activities. Supplies and other perks.
