Work Environment\n

This position is fully remote, providing you the flexibility to perform your duties from home while ensuring the safety and comfort of our clients. You will be part of a collaborative virtual team that includes healthcare professionals, caregivers, and community advocates who work together to deliver comprehensive services. Your schedule will primarily include business hours, from Monday to Friday, 8:00 AM to 4:30 PM, although there may be an occasional need for evening or weekend hours based on client requirements.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, considering emotional, mental, and social factors that may impact their quality of life.

• Develop and implement personalized care plans in collaboration with clients and their families to ensure their unique goals and needs are met.

• Provide counseling and emotional support, helping clients navigate life transitions, family dynamics, and health-related challenges.
\n
• Act as an advocate for clients, promoting their rights and ensuring access to necessary community resources and services.
\n
• Work effectively with an interdisciplinary team comprising nurses, caregivers, and other healthcare professionals to coordinate holistic care for seniors.

• Conduct regular home visits or virtual check-ins to monitor clients’ well-being and living conditions, addressing any safety issues that may arise.
\n
• Facilitate communication among family members involved in a client's care, helping to resolve conflicts and promote understanding.

• Educate clients and families about community resources, support programs, and social engagement opportunities to enhance their overall experience.

• Document all client interactions accurately, maintaining up-to-date records of assessments, care plans, progress notes, and communications.

• Stay informed on industry best practices, regulations, and ethical standards in social work to ensure compliance and deliver ethical care.

Patient Population / Client Focus\n

Your primary focus will be on seniors who are facing various psychosocial challenges, from emotional distress related to aging to navigating complex healthcare and living situations. You will be instrumental in ensuring that these individuals receive appropriate services and support to enhance their quality of life, advocate for their needs, and empower them in their healthcare decisions.

Qualifications\n

• Possession of a Master’s degree in Social Work from an accredited institution.

• A current and valid state license as a Master Social Worker (LMSW) in Texas.

• A minimum of two years of experience in a relevant social work setting, with a preference for experience in geriatric or community health contexts.

• Strong understanding of social work principles and the specific challenges faced by the elderly population.

• Demonstrated ability to develop rapport and communicate effectively with seniors and their families.

Compensation and Benefits\n
