Community Bridges, Inc. (CBI) is seeking a Case Manager to provide case management services in community and/or office settings across Arizona. The Case Manager supports patients and families by planning, coordinating, outreaching, advocating, and monitoring needs, and helps access health services, financial assistance, employment, education and social supports. Embraces recovery and a safe environment. Ideal candidates have a high school diploma or GED, 1–3 years GMH/SU experience, #J-18808-Ljbffr
