40 Position Summary
The Youth HIV Clinic Case Manager (Case Worker II) provides client-centered medical HIV case management services to youth ages 13–29, including comprehensive bio-psychosocial assessments and coordination of care with the multidisciplinary healthcare team. This position develops and implements individualized care plans that address educational needs, community resource referrals, and coordination of services. A positive youth development approach and shared youth decision-making are incorporated into all environments served.
The Case Worker II provides mental wellness screenings, crisis intervention and supportive counseling to assist clients and families in navigating health and psychosocial needs, diagnosis disclosure, confidentiality concerns, and adult systems transition planning and skills building. The role also coordinates access to internal and community resources in accordance with applicable local, state, and federal guidelines and procedures. The Case Worker II is expected to maintain the strict confidentiality standards required of all Washington University staff and work under the direction of senior staff. In addition to direct client services, this position includes one of the following lead or coordination responsibilities: 1. Providing a more intensive level of services, often community-based, requiring strong clinical skills and the ability to work independently;
2. Performing project coordination responsibilities, including special projects, reporting, and caseload assignments;

Primary Duties & Responsibilities:
Case Management Duties
- Conduct comprehensive, culturally sensitive bio-psychosocial assessments with clients and, as appropriate, family members or support networks.
- Synthesize program goals and assessed needs to develop individualized plans of care.
- Arrange and coordinate service referrals for clients and families as needed and monitor progress toward plan objectives.
- Refer clients and families to public benefits programs, as eligible.
- Independently manage a caseload by maintaining regular contact with enrolled clients and documenting all contacts and service coordination activities in accordance with program guidelines.
- Educate clients and families about the medical and psychosocial aspects of HIV/AIDS to support adherence to care.
- Adhere to all applicable state laws and department rules and regulations regarding HIV/AIDS programs.
- Maintain current knowledge of medical and psychosocial developments related to HIV/AIDS and related conditions and evidenced-based interventions for youth.
- Routinely interact and network with AIDS service organizations, providers, clinicians, and others involved in HIV/AIDS service delivery.
- Collaborate with case managers from other regions as needed when a patient resides outside of program service boundaries.
- Perform indirect, non-clinical tasks related to the position, including data compilation, required training, and attendance at staff and community meetings as assigned.
- Function as part of a multidisciplinary HIV Specialty Health Care Team and participate in scheduled case conferences to ensure coordinated delivery of care to and families.
- Educate clients and families on HIV/AIDS, medication adherence, and safer sex.
Specialty Clinic Case Management Duties
- Meet with all clinic patients ages 13–29 at each clinic appointment, regardless of enrollment in case management services.
- Offer mental wellness screenings to all youth, track efforts and outcomes, and coordinate referrals and other follow up for those with identified needs.
- Administer the transition assessment tool per program guidelines and partner with youth on identified areas for skill and knowledge development to support successful transition to adult HIV care
- Attend and actively participate in weekly follow-up case conferencing by providing clinical information and updates on youth.
- Meet with youth in the community to assist in facilitation of any needed medical or psychosocial services, and make reminder calls or texts to clients who may need additional support to address goals.
- Coordinate case management needs identified during medical visits.
- Serve as a liaison between the medical team and case management services and/or case manager.
- Engage with the multidisciplinary team during medical clinic sessions and follow-up.
- Follow up with clients who do not keep their appointments to identify and remove barriers to care, in coordination with team.
- Attend and participate in monthly Quality Improvement meetings.
Marginal/Nonessential Functions:
- Work collaboratively within Washington University Pediatric and Adult Infectious Diseases Clinic multidisciplinary teams and the Project ARK social services team.
- Attend monthly Ryan White community-wide case management meetings.
- Assist with other client support activities as deemed appropriate
