LMSW 2 Specialty Liaison

The LMSW 2 will serve as a Specialty Liaison, helping individuals in HCBS Waiver Ineligible Settings (hospitals, homeless shelters, psychiatric centers, etc) connect with needed services to assist with discharge planning. This position will also carry a case load as Crisis Mitigation Liaison for Region 3. This includes the 18 counties within Capital, Hudson Valley, and Taconic Districts. This position will work collaboratively with other clinicians and staff to assist with forensic/crisis/clinical needs across Region 3. Currently the unit is following over 400 cases, and the number is expected to grow. The LMSW2 will report to the Team Leader of this unit.

Primary duties and responsibilities:

• Serve as a Specialty Liaison, helping individuals in HCBS Waiver Ineligible Settings (hospitals, homeless shelters, psychiatric centers, etc) connect with needed services to assist with discharge planning.
• Oversight of crisis management/clinical consultation: This position will be responsible for crisis case coordination and clinical technical assistance to providers across service delivery milieu. This includes completing reports with clinical recommendations.
• Responsible for assigned crisis cases within the 18 counties of the Region 3 catchment area.
• Conducts various assessments and evaluations, completing reports with clinical recommendations.
• Responsible for accurate, contemporaneous tracking and timely follow up.
• Provide clinical consultation to voluntary agencies on Risk Management functions and provides follow up meetings and trainings as needed.
• Assists with the review of Adult Services Plans, Behavior Plans, Risk Assessments submitted for individuals who present with complex behavioral and mental health needs.
• This clinician will review, analyze, and professionally critique various clinical assessments including treatment plans, behavior plans, risk assessments, as well as provide an in-person screening of select individuals seeking placement.
• Duties will also include participation on various committees including but not limited to Risk Management Committees, conference calls, and other related meetings as determined by individual, and agency needs.
• This clinician will be working across the wide geographic region covered by region 3. Region 3 consists of 18 counties over 3 districts: Capital Region, Taconic Region, and Hudson Valley Region.
